Full Job Description

We are looking for individuals who will be able to oversees and coordinate patient care, educate patients and regarding various health conditions in long team home settings.

• RN should be monitoring, recording, and reporting symptoms or changes in patient’s conditions.
• Maintaining accurate and detailed documentations. Recording patient’s medical information and vital signs.
• Ordering treatment plans, interpreting, and evaluating diagnostic tests to identify and assess patient’s conditions.
• Modifying patient treatment plans as indicated by patient’s responses and conditions.
• Directing and supervising subordinates’ nurses and healthcare professionals.
• Coordinating with healthcare team members to assess, plan, implement and evaluate patient care plans.

Apply if you have these requirements:

• Current certificate of competence as a Registered Nurse with the College of Nurses of Ontario
• Minimum 2 years of nursing experience in a Rehabilitation/Complex Continuing Care Hospital or Acute Care Hospital preferred
• Current vulnerable sector check
• CPR and first Aid
• Certificate from a WSIB approved provider
• A copy of a 2-step TB test that is current
• N95 mask fit certificate due to COVID precautions
• Valid COVID vaccination Certificate
• A current copy of Immunization record – 2 step TB Test1, MMR, Tetanus, Hep B, Varicella, Flu Shot – annually.
